Our 150-bed facility opened in the fall of 2003 and provides a full array of rehabilitative therapies, transitional care, memory care, and long-term care in one location. We offer all private rooms, each with its own private bathroom. We are licensed for Medicare, Medicaid, and Veterans Affairs programs.
Our team has been recognized with a “Best of” award for Short-stay Rehabilitation in 2009, 2010, and 2011. We also received a LTC Link Spirit of Innovation Award for our Partnered Pathways program – an effort with area hospitals to enhance chronic disease management for patients with conditions such as congestive heart failure, COPD, and pneumonia.

HFM Receives Scholarship to National Conference

Holy Family Memorial was recently notified that they have received a scholarship for a representative to attend the Quality and Safety Roadmap in San Diego, CA in July. Only eight hospitals in Wisconsin received the scholarship.
The Quality and Safety Roadmap is a program offered in conjunction with the 21st annual Health Forum/American Hospital Association Leadership Summit that is designed to help hospitals achieve the pinnacle in quality and safety. Participants have the opportunity to hear experts share their stories of the journey to becoming a high reliability hospital. Participants also can engage with experts in an interactive session to discuss issues, challenges and achievements with peers. The program provides an opportunity to share ideas and gain new knowledge to create or enhance hospital’s quality and safety initiatives.
HFM earned the scholarship for a presentation they submitted on the success of their long term care medical home clinics (HFM clinics located within Shady Lane, Felician Village and Manitowoc Health and Rehab Center). The presentation highlighted how the clinics helped to cut down on hospital readmissions and emergency department visits.
Dr. Gary Schmidt, of HFM Family Medicine, was one of the founding physicians of this project. He believes having a continuous presence within the nursing homes allows physicians to be a part of the nursing home team, allowing for better communication between physicians and nursing home staff.
“The nursing home patients are not just patients being seen every other month, but they become part of your close knit patient population,” said Dr. Schmidt.
“Having a clinic within our nursing home strengthens the bonds between providers and hospitals and is key in helping reduce unnecessary hospitalizations,” said Julie Place, RN, Director of Nursing at Manitowoc Health & Rehabilitation Center. “For the patient, it provides an integrated health system and continuity in the delivery of care.”
